Orthopedics relies on good internal fixation of fractures in most cases, if not all, to attain sound union, good functional outcome, and early return to preinjury level of activities. While surgical approaches and techniques are learned and mastered over the years to perfection, the fracture union is not a guarantee. A working knowledge of fracture and implant biomechanics is critical to understand key concepts of fracture fixation for betterment of professional skills and patient outcomes. Many a times, it is seen that biomechanical concepts are bypassed to emphasis given in performing more number of cases and fine-tuning surgical skills that in many cases leads to failure of implant or the bone in achieving uneventful union.
An online learning platform, OSapp, is new offering from the house of AO foundation, Switzerland. The AO foundation is flag bearer in education, networking, innovation, and research in the field of trauma and musculoskeletal conditions. The OSapp is intended for better understanding of fracture fixation biomechanics as an interactive virtual learning program that teaches biomechanical principles through illustration, guided lessons, and three-dimensional model in silico simulation. The program was launched in AO Davos course (December, 2020) few months back and has topics covered in tutorials, principles, cases, and free configuration heads. Fracture type, fixation construct, and loading pattern are some of theoretical concepts as part of the module. Some examples such as plate working length, fracture gap motion, plate loading, and bone-plate contact are discussed which are important points for novice residents and early practitioners. Case-based teaching showing implant failure with causes being wrong plate, large fracture gap, or wrong working length selection is educational. Free configuration allows three-dimensional visualization of fracture and implant variables into play to learn key concepts. The working length and corresponding gap strain are better learned in three-dimensional mode of learning. The OSapp is going to be continually upgraded so regular review of important updates is necessary for continuous professional development. Under free configuration section, various aspects or parameters can be selected by user to evaluate the results for gaining key insights. Fracture variables such as location, type, gap size, stage of healing status can be chosen whereas for plates, type, size, screw type, screw configuration, plate elevation among others can be selected. Loading type (axial or torsion) can be also chosen for given fracture specifications. The best part is that it is free and users can learn basic biomechanics at their convenience. This platform should work as a good medium to enhance this critical skill for practitioners at each level.
